Jack M. Kozak, 52

Allen, Texas…A Mass of Christian Burial will be held Friday at 12:30 p.m. at St. Angela Merici Parish, with Rev. Kevin Peters officiating, for Jack M. Kozak, 52, formerly of Poland, who passed away unexpectedly in Denver on September 13, 2017.

Jack was born March 15, 1965 in Youngstown, the son of Jack and Marie Frazzini Kozak.

He was a 1983 graduate of Poland Seminary High School and later graduated in 1987 with a Bachelor of Science degree from Baldwin Wallace University.  Jack earned a law degree from Case Western Reserve University School of Law in 1990 and quickly passed the bar exam two months later.

He was the VP Compliance Counsel for Carrington Title Services in Plano, Texas.  Jack was a member of the Texas Bar Association and was a licensed attorney in 37 states.

Jack was a member of St. Jude Catholic Church in Allen, Texas.

He was an avid Miami Dolphins fan.  Jack loved his dogs and above all spending time with his family and grandchildren.

Jack will be deeply missed by his parents, Jack and Marie from Poland; his wife, Ester of Texas; two sisters, Susan Carney of Canfield and Nancy Reali of Hilton Head; three nieces, Ceara and Francesca Carney, and Shiann Reali; his children, Rudy Martinez Jr., Ernest A. Rosales, Lynette C. Martinez, and Ruby R. Martinez; and his grandchildren, Deliliah H. Martinez, Annette S. Martinez, Damien S. Martinez, Andrew Sebastian Rosales, Enessa Amerie Rosales, and Elise Alianna Rosales.

The Kozak family has entrusted their loved one to the care of the Wasko Funeral Home and will receive family and friends Friday from

11:00 a.m. to -12:30 p.m. at the church, prior to the 12:30 funeral mass.

In lieu of flowers, the Kozak family is requesting monetary donations be sent to St. Angela Merici Parish, 400 Lincoln Park Dr., Youngstown, Ohio 44506, in Jack's memory.
