John M. “Jack” Kozak, 84
YOUNGSTOWN…John M. “Jack” Kozak, 84 passed away peacefully surrounded by his family on Wednesday, May 20, 2026 at Hospice House in Poland, Ohio.
John was born May 2, 1942 in Youngstown, Ohio, a son of Michael and Mary (Swantek) Kozak.
He was a 1957 graduate of Campbell Memorial High School and earned his Bachelor’s degree in Engineering from Youngstown State University.
John married the love of his life, the former, Marie Frazzini on August 24, 1963 and they shared 58 years of marriage until her passing on March 12, 2022.
John grew up in the Youngstown area then moved to York, Pennsylvania for work in the 1970’s. John returned to this area in the early 1980’s.
He worked for Automatic Sprinklers, Inc. in York, PA as Vice President. He was owner of Knolls Paint Center in Youngstown and co-owner of Jated Distributers also in Youngstown.
John was of the Catholic faith and was a parishioner of St. Angela Merici Parish.
John enjoyed playing cards with his family especially poker and he also enjoyed trips to the casino. John was a car enthusiast and enjoyed swinging on the front porch at sunset. Above all he loved spending time with his family.
John will be deeply missed and always remembered by his two daughters, Susan (John) Carney of Canfield and Nancy Reali of Poland: three grandchildren, Ceara (Fady) Baky, Francesca Carney and Shiann (Kevin) Markiewicz; two great-grandchildren, Elliot Baky and Anderson Markiewicz; brothers, Alan (Lydia) Kozak and William (Karen) Kozak; sisters, Kathleen (Robert) Biasella and Victoria (Anthony) Gutierrez.
He was preceded in death by his parents; his wife and his son, Jack.
The Kozak family has entrusted John’s care to the Wasko Funeral Home in Campbell, Ohio. A Mass of Christian Burial will be held Wednesday, May 27, 2026 at 10am at St. Angela Merici Parish.
John will be laid to rest at Poland Riverside Cemetery in Poland, Ohio.
